Booking & Cancellation Policy
at BASE Spa & Wellness

A valid credit card is required for all appointments. This is to ensure your appointment on our schedule. We do require 24 hours notice to cancel. Please give us a call, email or text to cancel.

If less than 24 hours notice is given, 50% of the total appointment cost will be applied to the card we have on file.* If no notice is given, 100% of the total appointment will be applied to the card on file.** If the card so happens to not permit payment, we will require the balance to be paid prior to any future appointments being booked. If this occurs three times, you will be asked to prepay your appointments in full. Thank you for understanding.

Any Botox or Filler appointment is $25
Any consultation or follow up is $15
Laser, Facials and Skin Treatments: $50
PMU: $75
All others: 50% of the total appt cost

**No show:
Any Botox or Filler appointment is $50
Any consultation or follow up is $25
Laser, Facials and Skin Treatments: $75
PMU first session: $150
PMU second+ session: $100
All others: 100% of the total appt cost

Inclement Weather Policy

At BASE Spa & Wellness our clients are our first priority and we want to keep you safe as well as our employees. Our cancelation policy is that appointments canceled with less than 24-hour notice incur a cancellation fee.  During the winter months however, we are willing to make an exception to that rule.  If Porter County or nearby counties, is under a winter weather advisory/warning and you believe that it would be unsafe or impossible for you to travel to your appointment, you may reschedule within the 24-hour window at no charge. Additionally, in the event of inclement weather, BASE Spa & Wellness reserves the right to close and cancel appointments for the day for the safety of both our guests and staff.

Return Policy

We are happy to offer a return to the original payment method or in store credit within 14 days of purchase for items in their original condition, including packaging, with receipt or the ability for us to locate the receipt. We cannot guarantee we can locate it for you. We cannot accept damaged, altered, or used items for return.

All skincare marked “on sale” is final sale and cannot be returned.

Exchange Policy

We are happy to offer an exchange or in store credit within 14 days of purchase for items that did not work out for you with receipt or the ability for us to locate the receipt. We cannot guarantee we can locate it for you. We can only accept damaged or used items for exchange or in store credit. The product must be less than 10% used and still have a fully intact container and packaging.
